Important Information About HealthyExtensions

To take advantage of the HealthyExtensions discounts, all you have to do is contact the vendor, make an appointment (as needed) and identify yourself as a UNICARE¹ member. Be prepared to show your UNICARE ID card, if required, but some HealthyExtensions vendors may even extend the discounts to your parents and grandparents, even if they are not UNICARE members.

No referral from UNICARE or your physician is necessary. Because the vendor provides the discount and you are responsible for full payment of the discounted charges, there is no need to file a UNICARE claim.

While UNICARE is dedicated to making these discounted quality services available to you, we cannot guarantee the services or products offered by HealthExtensions vendors and make no claims, promises or recommendations regarding therapy procedures or outcomes. The vendor list was accurate at the time we last updated this page. The specific products and discounts, however, are subject to change or withdrawal without notice.

The HealthyExtensions Program is not a covered service under your health plan. The practitioners included in the HealthyExtensions Program are not members of UNICARE provider networks.
