Important Information About HealthyExtensions
To take advantage of the HealthyExtensionsSM discounts, all you have to do is contact the vendor, make an appointment (as needed) and identify yourself as a UniCare 1 member. Be prepared to show your UniCare ID card, if required, but some HealthyExtensionsSM vendors may even extend the discounts to your parents and grandparents, even if they are not UniCare members.
No referral from UniCare or your physician is necessary. Because the vendor provides the discount and you are responsible for full payment of the discounted charges, there is no need to file a UniCare claim.
