Plans at a Glance

This list is intended to help you compare UniCare plan benefits and reflects your share of costs for covered expenses after you have met any applicable deductible. When you use UniCare in-network (participating) providers, your costs are based on a specially negotiated fee for UniCare that may save you money. When you use out-of-network (nonparticipating) providers, your costs are based on covered expenses and often result in higher costs to you.

For a detailed description of coverage, benefits, limitations and exclusions, please refer to the applicable Certificate of Coverage. All plans feature a $5,000,000 per member lifetime maximum in benefits.

UniCare HSA-Compatible Plans

A Health Savings Account (HSA) is a savings account that allows eligible individuals to save money that can be used to pay for qualified medical expenses on a tax-free basis. All HSA-compatible health insurance plans are required to meet certain federally mandated deductible and out-of-pocket requirements.

UniCare offers a variety of High-Deductible Health Plans, each with a different annual deductible amount for both individuals and families.
