Welcome Group Plan Members
As a group plan member, your health plan is provided through your employer and administered by UniCare.
If you want information about your specific plan, consult your benefit manager at work or call customer service at the toll-free number on your identification card. Up-to-date information about your employer's benefit plan is kept on our computer system and is readily accessible to our customer service representatives who can answer many of your questions with one easy phone call.
Customer Service can give you general information about your health plan or explain specific benefits, including:
- any copayments, deductibles or coinsurance;
- how to get care away from home;
- what to do in an emergency;
- how to file a claim or give you the status of a claim.

- It gives the name of the network through which you can receive quality care with the least out-of-pocket expense.
- It provides useful telephone numbers, including those for customer service and managed care services, who must usually pre-certify services such as inpatient hospital admissions.
- It will sometimes indicate the copayment you must pay physicians for each office visit.
- It will sometimes indicate if you have other than medical coverage through UniCare, such as dental or prescription drug.
